By digitally signing the box below, I wish for Petmania Vets to confirm the procedure I have requested for my pet.

If my pet is unable to attend the appointment as arranged for any reason, I understand I need to give 48 hours notice in writing to info@petmaniavets.com.au.

I understand if my pet is unable to undergo the procedure as confirmed and I do not give greater than 48 hours notice, a cancellation fee of $150 is payable on the day instead.
